6.Guidelines to fill the online Application form:
Note: Since the Applicant should upload their photographs in online
application entry, the applicant should scan their passport size photograph
(Snapped after 01/04/2018 with Name & Date on the photograph) in .jpg
format and make available for uploading with the following specifications.
Width = 1.3 inches, Height = 1.6 inches, Resolution = 200 X 200 dpi and the file
size should be less than 100kb.
Select the Course – ¨B.Sc Nursing/ BPT /BPO
1. Name of the Candidate: Fill the name in BLOCK LETTERS as per 10th standard
Marks Card.
2. Mother’s Name: Mother’s name in full should be filled.
3. Father’s Name: Father’s Name should correspond to the form and spelling in the
10th standard Marks Card.
4. Date of Birth: The Date of Birth should correspond to the entry in the 10th Marks
Card in dd/mm/yyyy.
5. Place of Birth: Select the District in which you were born.
6. Gender: Select from the drop down list Female /Male.
7. Nationality: Enter the Nationality.
8. Qualifying Examination: PUC (10+2) or Equivalent Examination
9. Marks obtained in II PUC (Physics, Chemistry, Biology)
10. Domicile of Karnataka : Select form the drop down list Yes/No.
11. Category: Selective the respective category and enclose the certificate obtained
from the Tahashildar of Karnataka

Religion, Caste: Mention the religion, caste and sub-caste you belong to
Postal Address: Please furnish full postal address in BLOCK LETTERS. Name of
the City, State and Pin Code should be clearly mentioned. Telephone with STD
code/ Mobile and e-mail ID may also be mentioned.
Do you claim reservation under Physically Handicapped quota: Select Yes or
No
Details of fee: Enter Amount, ID Number, Date and Bank Name where Amount
is credited. (Rs.250/- for SC/ST & Category – I and Rs.1000/- for General &
OBC categories).
Reservation claimed under HK region: yes/no
After filling the online application form press SUBMIT button, if you are sure that
all the data entered by you is correct. The system will ask for uploading
photograph and the scanned photograph of .jpg format file with the above
mentioned specification has to be uploaded. An Application Number will get
generated and please record it for your future usage. Take the printout of the filledin
application and paste the recent passport size photograph which is scanned and
uploaded in the space provided. The print out of the filled-in application duly
signed by the candidate be sent to University along with the required documents
within the stipulated time.
7. Merit List:
The Course wise merit list will be announced on the KEA website as per
calendar of events.
8.Registration Fee: Mode of Payment:-
The prescribed registration fee should be remitted through payment
gateway by using Debit or Credit Cards (Master or Visa).
Candidates are hereby informed that payment SHOULD NOT be made
through Demand Draft or any other mode of payment other than what is
mentioned above.
• General and OBC candidates Rs. 1000/-
• SC, ST and Category - I candidates Rs.250/-
Note: Registration fee is not refundable under any circumstances.

The following attested copies of the documents to be submitted at the
Time of Verification
1. S.S.L.C. Marks Card (For age proof and parent’s name).
Birth Certificate/Cumulative Record/domicile certificate 07 years
2. marks card from 01-10th standard any other certificate for proof of
Date of Birth, Place of Birth.
PUC Marks Card. If PUC studied in other than PUC board of Karnataka
3. they should submit eligibility certificate issued from Rajiv Gandhi
University of Health sciences.
Category Certificate issued by the Tahashildar for candidates claiming
reservation under different categories in the prescribed form along with
Validation of Caste Certificate issued by concerned scrutiny committee of
that District

5. Disability Certificate if claimed under Physically Handicapped quota.
6.
Certificate under article 371-J for reservation under HK region issued by
competent authority.
7. Any other certificate.

Beware of touts, brokers or middlemen
Few persons may approach you saying they have known people in KEA who
guarantee seats through unfair means during counseling. Counseling will be
conducted and distributed as per the merit of the candidate and option entry
exercised by the candidate. Hence, candidates and parents should not trust any
